Appointments
Appointments are taken
by phone at 615-824-7220. If your situation is an emergency, we
will make every effort to schedule you as quickly as possible.
Dr. Sullins is
available:
Monday through Thursday:
8:00am-5:00pm
Friday: 8:00am-12:00pm
Please note the
following:

Dental insurance cards
should be brought to appointment.

All patients under the
age of 18 years must be accompanied by a parent or guardian at the
initial appointment.

When
scheduling an appointment, please be sure to indicate any special
medical needs (i.e.: artificial heart valves, joints, heart murmur
or anything requiring the need for pre-med).

If you
have had X rays at a previous dentist or referring specialist, you
may request that they be forwarded to us.

Insurance and Financing We
participate with most major insurance, including Delta Dental and Blue
Cross Blue Shield dental plans as well as TennCare. As a courtesy, we
will file your insurance claims. However, your estimated portion
is due when services are rendered.
For your
convenience we accept MC/Visa, Discover, cash or check. We also
offer financing through
CareCredit with a variety of payment options to fit your
needs.
